Application Checklist
J-1 Trainee/Intern Application Information Requirements
- Copy of Passport, all pages included
- Copy of Accompanying Dependents’ Passport ID pages (if applicable)
- Resume with employment and educational history
- Educational transcripts with English language translations as needed from post-secondary education outside of the United States
- Post-Secondary diplomas or academic certificates with English language translations as needed (intern)
- Proof of full-time enrollment in post-secondary academic education if not graduated (Intern only)
- Employment verification letters with English language translations as needed (trainee only)
- With university degree, one year of experience directly related to proposed training
- Without university degree, five years of experience directly related to proposed training
- Signature of the J-1 Trainee/Intern applicant agreeing to the terms of the exchange visitor program.
Information on the Host Organization Information
- IRS Employer Identification Number
- Date of Incorporation (host must be in business more than 24 months)
- Number of employees (minimum number of six full time permanent U.S. residents staff on site)
- Annual revenue
- Workers’ Compensation Policy Carrier and Number
- Signature of a representative agreeing to the terms of the exchange visitor program.
The Exchange Visitor Financial Support and Budget Form
- Information on probable exchange visitor expenses
- Information on financial support provided by all sources
The DS-7002 Training Plan (download and upload pdf)
- A separate DS-7002 signature page is required for each site of activity and for each Direct Supervisor
- Description of the program
- Host organization background information
- Overall program objective
- Description of overall skills and knowledge the J-1 exchange visitor will acquire, how the skills are taught, and how they pertain to individual’s ultimate career goals
- Description of orientation to workplace and local community
- Description of cultural exchange opportunities
- Description of planned cultural opportunities
- Description of direct supervisor’s qualifications
- Description of evaluation measures
- Signature of both the J-1 intern/trainee and supervisor(s) (don’t sign for sponsor)
- Uploaded complete and signed document
Insurance Declaration
- Insurance Enrollment for each person insured under American Immigration Council identified group policy
OR
- Insurance Confirmation if opting out of American Immigration Council identified plan
On-going Exchange Declaration
- Signature of the host organization representative and the J-1 intern/trainee
